Multi-Agency working


Multi-agency integrated support includes the CAF, lead professional, effective information sharing and in the future, supporting tools such as ContactPoint and National eCAF. Multi-agency working aims to support children and young people earlier, to ensure they meet the five ECM outcomes.

Multi-agency working relies on all practitioners maintaining a good working relationship with each other improving the co-ordinated services provided to children, young people and their families.

We have a Multi-Agency Panel (MAP) which meets on a monthly basis to discuss the needs of children and young people who may benefit from  multi-agency support. Before the panel can offer support a Common Assessment must be completed, and the child, young person or family have to give their consent for the CAF form be presented to the panel.

Panel members are represented from across health, education, social care and invite other professionals with areas of expertise relevant to the concerns raised on the CAF form.

Meeting dates

Our panel meet every second Wednesday of each month, from 12 noon -2pm.
